the loss of a loved one. Listed below is some useful information on where to register a death and what you might need in order to do this. Remember, we are always here at the end of the phone to answer your questions or lend a sympathetic ear.

When going to the Registrar please ensure that you take:-
- A death Certificate, (From a Doctor or Hospital)
and if possible:
- Birth Certificate (Date & Place of Birth)
- Medical Card
You must be able to state accurately the following particulars:-
- Date and place of death
- Residence of deceased
- Full name and surname (Maiden name if a woman)
- Correct age, date and place of birth
- Occupation
- Whether the deceased was in receipt of a pension or allowance from Public Funds
- If the deceased was married, date of birth of surviving spouse
You will obtain from the Registrar:
- Disposal Certificate (Green Copy – Funeral Directors)
- White Copy (For DSS – Pension)
- Extra Copies (For Probate/Insurance purposes)
- Tell us once form (for Blue Badge, Council Tax)
